I voluntarily allow my child to take part in Northwest Florida Soccer Academy LLC ("NWFSA") practices, games, travel and events. Release. To the fullest extent the law allows, I release NWFSA, its owners, coaches, staff, volunteers and sponsors from claims arising from my child's participation, except for gross negligence or willful misconduct. Medical authorization. If I cannot be reached in an emergency, I authorize NWFSA staff to seek emergency medical treatment for my child. I am responsible for the cost of that treatment. I am the parent or legal guardian of this player and I have read and agree to this waiver.

I can withdraw this permission for future use at any time by writing to academy@nwfsa.com.
